It's good to back and see this thread still half alive, chmod -R 755 Quake3-UrT.app is confirmed to be working on my MacBook Pro with the latest version of UrT 4.2.020. (Delete the text "Quake3-UrT.app and simply drag the application into terminal =) ) (It took me a while to figure this out

Back when I played on a mac, I found that, sometimes UrT crashed on startup because r_fullscreen was set to 1 for some reason even though I had it in window mode. I changed it to r_fullscreen "0" and I never had that problem again.

I have Mavericks and am using the most recent build of the game, but I get the "black flash then crash" when I try to start the game. I noticed in the Application support folder "Quake 3" it only generates the qkey file when I try to start the game and nothing else. Can someone upload the contents of what is supposed to be in the folder somewhere? I feel like it's a problem with some setting but I don't have anywhere to change the settings because they're not being generated.

Found a fix after some searching online....it wasn't generating though files because the game wasn't starting. It wasn't starting because I needed to put this in console:
/Users/Home/Applications/Urban\ Terror/Quake3-UrT.app/Contents/MacOS/Quake3-UrT.i386 +set r_ignorehwgamma "1"
For others having trouble:
1) Open terminal, drag the UrT app into the window. Append
/Contents/MacOS/Quake3-UrT.i386 +set r_ignorehwgamma "1"
to it.
2) It should start. Hope this helps someone else. I can't take the credit for it, combined info from
