I had been playing over the last few days using a mouse which is 15 years old and have been really struggling. The buttons etc. certainly aren't the same as what I was once used to in using a steelseries sensei (raw)... and I had assumed that the difficulty I was experiencing in performing as I once did was a combination of a 'bad mouse' and just plain rust. Admittedly, I had set my sensitivity a little too high (overambitiously)... but it dawned on me today that I had installed a new operating system!
Which meant, that my MarkC mousefix was no longer working... which meant that I had MOUSE ACCELERATION.
I felt comfortable again almost right away after applying the MarkC Mouse Fix and was back to my old form in no time.
I highly recommend using this if you have not yet heard of it already.

Edit: Is this the same as having raw input (which I am told can be achieved by using in_mouse 2 followed by an in_restart), I wonder? In any case, it seems to remove mouse accel entirely, in-game and out... which... I imagine is preferable to just using raw input in-game... your choice.
