When I /connect to a server or watch a demo and then join a server through the gui I will be missing some textures, breathing sounds, and sometimes my mouse doesn't work normally. The mouse will be "boxed in" unable to turn all the way. Also, the flags are different, American and USSR flags instead of my custom player colors. Restarting the client fixes the mouse issues. Restarting the client also fixes the texture & breathing sound issues when I join a server using the gui. Restarting the client does not fix texture & breathing sound issues when I use /connect.
Is this a config problem? Is there a way to fix?

You try to edit anything of the folder q3ut4?
You installed correctly or update it fine?
Because most of this problems are generated for bad installs or when any one try remove or edit files from the q3ut4 folder.

Try launching the game from console, reproducing the odd behaviour and see if you get any interesting output. Feel free to gist or pastebin it too.

I've only ever edited my config. I have symlinked the Quake-UrT.i386 binary to /usr/local/bin though.

It's not reproducible. Today I was playing uptown and everything was fine but when I disconnected and reconnected the lights were solid black. I disconnected and reconnected through the gui so I'm wrong about the demos and /connect. The missing textures, sounds, and mouse are just random I guess.
The output looked fine except for stuff like:
^3WARNING: 'textures/urban_terror/glight1.tga' TGA file header declares top-down image, ignoring
It's just a warning I know, but again the lights on Uptown were solid black.

using /connect I got stuff like:
^3WARNING: invalid cull parm 'front' in shader 'textures/tub/sn_rail1t'
and nothing indicated why I didn't have breathing sounds.

I might reinstall the game.

I don't know, but the custom flags sound like an issue that could be caused by servers downloading and installing messed up pak files (although that shouldn't be possible). Reinstalling will probably fix it, or try to clear out your downloaded custom pak files?

Thanks, I'm reinstalling right now

Just remember the map downloads are all in $HOME/.q3a/q3ut4/ and $HOME/.q3a/q3ut4/download/, which won't get removed unless you manually delete 'em. I can't believe you're using the 32 bit client :P

The boxed in mouse (cursor in a box) is a known bug which affects Ubuntu and other distributions. It is only present on 32bit binary. Thinking about it maybe it was 4.1.1 affected by this...

Use the 64bit if possible and you will be fine.
The Flag thing is a screwy map. Probably the one Orbit said.

I created a list of maps that break textures in standard maps, I'm at work now so i dont have it but if I recall, as an example
ut4_ village breaks sky texture in abbey
ut4_alcantarilla breaks puddle texture in Ramelle
ut4_Blackmarket breaks light texture in uptown.

I'll confirm when I get home.

Confirmed.
ut4_alcantarilla_bc3.pk3 --- BREAKS puddle texture in ramelle
ut4_blackmarket_beta3.pk3 --- BREAKS light texture in uptown
ut4_village.pk3 --- BREAKS skybox texture in abbey

Also The mouse texture bug is a 4.1.1 bug that I could not duplicate with 4.2.023.
