Try the '2.0 Headphone (Hybrid HRTF)' option. It'll make you know closed eyes if something is front or back.
It eats some cpu power.

Yes! That's what I'm talking about here. By using 2.0 headphone HRTF options, the software makes sounds sound differently depended on where they come from. Regular q3 sound makes sounds sound the same so it's a mess.
It's something like if you face firing front it's a bit more 'bass' than being behind you.

It uses some more than normal q3 sound. But, newer versions of it let you just use 8 channels of audio (number of channels are responsible a lot about cpu usage) with little audio degradation while still sounding nice and working. Details are in the linked thread.
