First of all. When the player is in throw mode, the knife does not show up as fliped.
You can see the even though the status near the ammo sais 'Throw' it is not in that position
Second, once the knife has been thrown into something, it does not fit the orientation that it was thrown as.
The blade in this picture should be facing the other way judging by the oriantation it was thrown. (The knife was thrown from the left)

I think the way the medic pack devours the gun is a bigger graphical problem ;)
But yeah, it would be nice if this could be fixed.
As for that second thing, I'd have never even noticed this unless you brought this up, good find.

Afaik point one will get fixed from Frankie V.
In Point two the first person knife should look away with the sharp side from the player but that was probably done for an more esthetically look and to fit the animation.
