I tried doing bind MOUSE4 "ut_itemdrop kevlar" but it doesn't do anything but when I do Mouse3, it works. I also tried trying to bind drop medkit with mouse4 but that doesn't work either.
Any help?

Or use one of the community builds that supports this mouse keys by default (mitsu's optimized / p5yc0's dmaHD).
