Well, I myself was confused. A bit of a dilemma, keep the scale of my map at an old scale like which was present at 4.0.
Or build a map based on a scale meant for 4.0
Well I made a little map preview, and included two rooms,
room 102: old scale
room 101: new scale
Basically I just want to you what you guys would prefer.

Heres a screenshots of the small preview:

And heres the download link:

to load up the map type:
/map ut4_hospitalsc

Please bear in mind this is my first map . Also constructive criticism is welcomed

The doors seem a little too narrow and short.

DESK

new scale - too tall
old scale  - OK

SOFA

new scale - too big
old scale - OK

DOORWAYS

old scale - too thin and i keep getting caught on the door. the height isn't bad, i think 84-88 units is about right for an internal door height.
new scale - still too thin and I'm still getting caught on the door when running through; maybe you could add a triangular clip next too the door so people don't get stuck. height is a bit too much. I'd say 64 units is decent width for a door. The smallest doors I used in mandolin were 56wide by 84 high, and they didnt have anything partially blocking passage through them.

Overall - old scale is better but the doors need reworking.

Additional, I hope your only using ambient light/min light because this is a test map, its making the map look all washed out and monotone.
